• Medicare or the veteran affairs card.
• Have your insurance information with you.
• Current medication list

You may be able to avoid repeating a study if your information is here. You may also arrange to have the records sent here prior to your appointment.

To cancel an appointment:

Please call on 03 9885 7177 during office hours and let us know at least 24 hours in advance so we can give that time to another patient.
